OSU faculty and staff may request funds to support outcomes
assessment (for undergraduate and graduate programs)
or assessment in other program areas including entry-level
assessment, mid-level or general education assessment, and
assessment of programs aimed at enhancing student learning.
Budget
requests for fiscal year 2011 are due June
4, 2010. Submit budget requests as an email attachment to your College Assessment Coordinator and to the Office of University Assessment and Testing (jeremy.penn@okstate.edu).

Describe each assessment activity to be funded, including
the name of the assessment method. |

Describe what is being assessed (for outcomes assessment
requests, the request may simply refer to the unit's
current assessment plan for this information). |

Describe who will participate in the assessment. |

4.
Provide the amount requested for each assessment activity. |
| |
5. NEW FOR FISCAL YEAR 2011: If funds were provided for fiscal year 2010, describe how funds were used and a brief overview of the success of those assessment activities. If funds were provided for fiscal year 2010, funding for fiscal year 2011 is contingent upon submission of an Annual Assessment Report by September 24th, 2010 - failure to provide a report will result in forfeiture of all outcomes assessment funds for fiscal year 2011. |

6.
Include a completed budget form. Accounting subcodes
are needed to correctly transfer funds; your unit's
financial officer can help complete this form.

Special
Projects Requests. The Assessment Office also provides
support for 'special assessment studies' that are usually
studies aimed at evaluating the effectiveness of OSU student
programs and services at the college- or university-level.
